Botulinum toxin aka Botox works by temporarily blocking the nerve that activates a muscle. The area of the nerve that Botox affects is called the presynaptic area. The presynaptic area is before the synapse. The nerve stimulates the muscle at the synapse. Botoulinum toxin binds to the presynaptic motoneuron and then is internalized. It acts on a zinc-dependent endoprotease to inhibit acetylcholine release. Acetylcholine is responsible for stimulating the muscle. Hence, no acetylcholine, no muscle movement. If a patient is zinc deficient, botulinum toxin will not be internalized and will not work as effectively.
