Have you ever looked in the mirror and thought, "I look tired even when I feel rested?"

For many people, the answer may be related to changes around the eyes.

The eyes are one of the first areas where facial aging can become noticeable. Under-eye bags, excess upper-eyelid skin, hollowness, and changes in the brow can all contribute to a tired appearance.

But lack of sleep isn't always the culprit.

Why Do Under-Eye Bags Develop?

Under-eye bags can have several causes.

As we age, tissues that once supported fat around the eyes can change. Fat may become more prominent, while the skin and supporting tissues become less firm.

Genetics also play an important role. Some people develop prominent under-eye bags at a relatively young age.

Other factors, including allergies, fluid retention, and lifestyle, can temporarily make the area look more swollen.

What About Under-Eye Hollows?

Not everyone with a tired-looking eye area has excess fat.

Some patients have hollowness beneath the lower eyelid, which can create a shadow or transition between the lower eyelid and cheek.

This is one reason individualized evaluation is important. Adding volume to a hollow area and reducing excess tissue are very different approaches.

What Is Blepharoplasty?

Blepharoplasty, commonly called eyelid surgery, is designed to address certain concerns involving the upper and/or lower eyelids.

Depending on the patient's anatomy, surgery may address excess skin, fat, or other structural changes contributing to an aged or tired appearance.

The goal isn't simply to remove as much tissue as possible. Overcorrection can create an unnatural appearance, which is why conservative, individualized planning is important.

What About the Upper Eyelids?

With age, excess upper-eyelid skin can develop and may contribute to a heavy or tired appearance.

In some patients, changes in the brow also contribute to the appearance of the upper eyelid.

A facial plastic surgeon can evaluate the entire upper face rather than treating the eyelid in isolation.

Can Skincare Fix Under-Eye Bags?

Skincare can improve certain aspects of the skin around the eyes, such as texture and fine lines.

However, creams generally can't reposition deeper tissues or eliminate significant excess skin or prominent fat.

Understanding what's actually causing the concern is the first step toward choosing an appropriate treatment.

The Bottom Line

A tired-looking eye area isn't always caused by lack of sleep.

Genetics, volume changes, skin laxity, fat position, and brow position can all contribute.

If you're bothered by changes around your eyes, a facial plastic surgeon can evaluate the underlying anatomy and discuss whether skincare, nonsurgical treatments, eyelid surgery, or another approach may be appropriate.
