Middle Eastern Rhinoplasty

It is not possible to identify an entire region with one type of nose.  Instead there is a range of noses found within this area.   Middle Eastern noses may have thicker more sebaceous skin, ptotic or drooping tip, wider nasal tip and a dorsal hump.  The goal of rhinoplasty for any ethnicity is to improve the appearance of the nose so that the the rest of the face is more attractive.  Some patients of Middle Eastern origin will complain that their nose is a dominant portion of their face and desire it to be featured less.

Interestingly, the structure of each nose can be quite different and depends on the stiffness of the lower lateral cartilages and thickness of soft tissue.  The soft tissue envelope of the nose must be accounted for.  In some patients of Middle Eastern origin, there is a thicker subcuteanous layer where actual fat is present.  Removal of the fat can help provide definition to the nasal tip.

Figure 1.  Above left and Above right demonstrate preoperative photograph with long nose and nasal hump.  In addition, patient had an overprojected nose with a bulbous nasal tip.  The patient's nasal tip was elevated and the nasal tip was further defined despite the patient's thick nasal skin.  The nose now looks less dominant and fits in better with rest of the facial features.  This patient did not want her nose to look artifical or overly sculpted.

The stiffness of the lower lateral cartilages also plays a role.  Firmer cartilages tend to provide more definition to the nasal tip, while soft cartilages will give the nose a more rounded appearance.  A ptotic or drooping tip often needs more support to the nose.

There are several approaches to the Middle Eastern nose.  The best approach really depends on the patient's individual anatomy.  Some patients are clearly better candidates for a closed technique (scarless), while others will benefit from an open technique.  In addition, some patients will not like the overall size of their nose.  Other procedures to reduce the overall size can play a role in allowing the soft tissues of the nose to redrape and make the nose a less prominent feature of the face.

A key component of the rhinoplasty is the planning which takes preoperatively and during the consultation to ensure that the patient and doctor are on the same page.
