What are the differences in eyelid rejuvenation between men and women?
Men need more conservative approaches than women. While women can wear makeup to cover up and eyelid scars, men do not have that luxury. In addition, the classic handsome male brow is full, low and thick, while a female brow can vary from full to thin to low to gently arched.
An occidental women's eyecrease can vary from medium (8 mm) to higher levels (12 mm). An occidental men's crease should be much lower from 7 to 9 mm. This subtle difference makes a men's eye appear much more natural.
In additon, while most women like a "clean look" with smooth skin and fat conserved lids, a "clean look" can look too done in a male patient. My clientele do not want any signs of plastic surgery. If a CEO looks like he has had eye surgery, he will feel self conscious and may actually lose confidence. In general, I usually prefer the most natural look possible in light of the fact that men can not wear makeup and the stigma of having plastic surgery can be detrimental and just flat out look bizaare.
How do you approach the male brow?
Men's brows can be difficult problems to manage. An overly raised brow, or surprised look, will be a clear sign that plastic surgery has been performed in a male. The goal is to maintain or slightly raise the brow to a natural position.
Endoscopic forehead lifts have some merit, but must be used in caution with male patients. First of all, endoscopic forehead lifts can raise the hairline. All men do not want their hairline to start farther back then it should. Incisional approaches can lead to numbness and tingling in male patients and risk visibility if future hairloss is anticipated.
Chemodenervation with botox is a conservative approach that is effective and simple procedure for most men. It is a non permanent solution. Fat transfer in beneath the brow, within the brow fat, can also lift the brow and is much more predictable around the eyes than in other locations. Finally, transblepharoplasty approaches combined with endscopic approaches can be effective in certain men.
